Tips for Organizing Your Health Records

  • Keep a physical folder with copies of insurance cards, medication lists, and after-visit summaries from each appointment.
  • Use your phone's notes app to log symptoms, questions, and medication changes between appointments.
  • Ask for after-visit summaries at every appointment — most FQHC electronic health record systems can print one at checkout.
  • Request access to the patient portal if your FQHC uses one — lab results, messages, and appointment notes are typically available there.
  • Take photos of prescription bottles so you always have the medication name and dosage on your phone.
