Where Does FQHC Quality Data Come From?

Every FQHC submits annual data to HRSA through the Uniform Data System (UDS) — a standardized reporting system covering clinical quality, patient demographics, staffing, and financial data. This data is publicly available and forms the basis for quality comparisons.

Top Tier

Health Center Quality Leader (HCQL)

Awarded to the top 33% of FQHCs nationally based on clinical quality performance across key measures. This is the most prestigious HRSA recognition and indicates consistently high performance.

Performance

Gold, Silver, Bronze Medalist Awards

Awarded for achieving national benchmarks in specific clinical areas. Gold = top 10%, Silver = 75th percentile, Bronze = meeting the national benchmark. Areas include diabetes control, hypertension control, depression screening, cervical cancer screening, and more.

Specific

CHQR Badges (Clinical Health Quality Recognition)

Granular badges for excelling in specific clinical domains: Diabetes Health Leader, Heart Health Leader, Behavioral Health Leader, Cancer Screening Leader, Prenatal Care Leader. A clinic can hold multiple CHQR badges.

UDS Data: What's Publicly Available

Through the HRSA UDS Mapper and Data.HRSA.gov, the public can access:

  • Annual patient counts by payer type (Medicaid, Medicare, uninsured)
  • Clinical quality measure performance
  • Service sites and grant amounts
  • Quality award designations by year

How to Use Quality Ratings When Choosing a Clinic

  • Look for HRSA Quality Leader designation as a strong positive signal — this puts the clinic in the top third nationally.
  • Check CHQR badges relevant to your health needs — if you have diabetes, look for a Diabetes Health Leader badge.
  • Don't over-index on awards — a clinic without awards may still be excellent. Consider patient reviews, wait times, language access, and services offered.
  • Compare within your region — rural FQHCs often score well relative to their context even if national rankings are lower.
