READ BEFORE YOU SIGN ANY CONSENT FORM FROM THESE CROOKS!!!! See the attached forms I refused to sign. They tired to trick me into signing these on their I pad. Just because I’m homeless doesn’t mean I’m uneducated. SOMEONE AUDIT THIS PLACE! I was attempting to be seen for an antibiotic, that is it! These documents show you participate in psychiatric care, you allow people to have authority over any money you receive, over your medical care, it even shows that you participate in group therapy. Absolutely disturbing. REPORT THESE PEOPLE!!!
You get what you do or don't pay for. Lots of misinformation and indirect communication by phone and in-person, apparently even between people who work there. Lots of conflicting information, lack of follow-through. You have to do a lot of the work and information gathering if you don't want to come back 7 times to complete many tasks, apparently. The people in the pharmacy are pretty great, though they did just switch to a new system and told me my as-needed refills needed a doctor's approval this week by mistake. This was eventually corrected by me calling the pharmacy (and speaking to the pharmacist) after coming back 3x to refill a nicotine gum prescription.
They started to do my dental work and then they just quit and it's been a year since they even tried to call me try to fix my mouth anything they have before she just left me hanging and didn't care that they pulled all my teeth and didn't give me dentures or nothing and here it is a year later and they still ain't trying to do anything when I talk to him they kicked me out of their clinic and told me do I need to find my own dentist now. Didn't care about my house or anything when I went back in to talk to them about my dentures and get my mouth fixed they kicked me out and told me not to come back

Wonderful services and experience with Fourth Street Clinic. Shoutouts to: Sage, Josh (phlebotomist), Leah (window 4), and also the receptionist who did my registration at the front desk, and the nurse who walked me back to the room, but who's names I sadly forgot. Dr. Chris Belknap was my Physician and his demeanor really put me at ease talking with him. He was personable, had a willingness to listen, curious, and empathetic about my health concerns. All of the staff of whom I mentioned were very kind, helpful, efficient, professional, and went above and beyond their duties. I had a pleasant experience with my visit and I HIGHLY recommend this clinic. *Especially for the homeless as they don't make you feel judged, profiled, or awkward, unlike other places I've been. 🙌 And with that being said, the building, waiting room, and examination room were clean, peaceful, quiet, and not what I expected from this clinic.
